Patient Information New Patients... New patients registering with the practice will be asked to make an introductory appointment within one month with a Practice Nurse. Home Visits... Due to pressure on Doctor's time, home visits are reserved for patients who the Doctor feels are either medically unfit or too infirm to attend Surgery and children with acute illness. Please make requests for home visits as early as possible in the morning so that the Doctors can organise their visits. The Receptionist may request brief details in order to assess the urgency of the request. For medical emergencies, out of hours calls are answered by the Doctor Answering Service (01248 674421). Repeat Prescriptions... Please submit you prescription slip by post or in person and allow 48 hours for your request to be processed. Click Here to order your repeat prescription! Change of Details... Please inform us of any change of address of personal details. This will help us keep you records up to date. Complaints Procedure... Information is not passed on without your consent unless it is within the confines of the NHS, by legal framework, or is in the public interest. Certain anonymised patient data may be shared for the purposes of public health and audit, research teaching and training. This practice is registered under the Data Protection Act. It is a practice and legal requirement that all staff maintain confidentiality of patient records. Freedom of Information... The Freedom of Information Act 2000 obliges the Practice to produce a Publication Scheme.
