Violent & Abusive Patients

 

The Practice operates a zero tolerance policy towards violence and abuse against Doctors and Staff.

 

Change of Details

 

Please inform us of any change of address or personal details that will help us keep your records up-to-date.

 

Blood Tests

 

If you have blood tests done at the surgery, it is essential that you telephone us within a week to get your results.

 

Disabled Access

 

Both Coed Y Glyn Surgery and Glan Menai Surgery have been adapted to suit the requirements of disabled patients.

 

Data Protection

 

Your medical records are held in the strictest confidence. Information is not passed on without your consent unless within the confines of the NHS, by legal framework, or is in the public interest. Certain anonymised patient data may be shared for the purposes of public health and audit, research teaching or training. This Practice is registered under the Data Protection Act. It is a practice and legal requirement that all staff maintain confidentiality of patient records and inforamtion.

 

Freedom of Information

 

The Freedom of Information Act 2000 obliges the Practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the Practice intends to routinely make available. Available on request at the Practice.
